Inpatient Rehabilitation Services
The mission of the Center for Rehabilitation at Westlake Hospital is to:

Promote the maximal attainment of physical, self-care, cognitive, communicative, psychological, spiritual and psychosocial status in patients with functional impairments.

Provide the patient with an interdisciplinary program that embraces the core values of compassion, accountability, respect, excellence and service.

Provide a system for monitoring and improving functional outcomes, patient satisfaction, and program efficiency while providing cost effective services.

Our Team
Each patient will work with a designated rehabilitation care team to design an individualized treatment plan based on their needs and goals.
Meet our team of specialists:
Physiatrist A physician specializing in physical medicine and rehabilitation, who manages the medical needs and oversees the care plan for each patient to monitor progress and ensure the treatment is appropriate.
Nursing Staff reviews the care, consults with the physicians and reinforces the self-care activities learned in the therapies.
Physical and Occupational Therapists conduct thorough evaluations of individual skills, coordinate the development of care plans and individualize treatment programs.
Speech-language pathologists assess, diagnose, treat, and help to prevent disorders related to speech, language, cognitive-communication, voice, swallowing and fluency. On average, patients receive three (3) or more hours of therapy daily, up to six days a week.
Recreational therapists provide treatment services and recreation activities for individuals with disabilities or illnesses.
Social workers help patients and their families move through the continuum of care, assuring that you receive the most effective and appropriate level of care. Working with you, your family, your insurance carrier and your health care team, the case manager will help you formulate a discharge plan that is most appropriate for your situation.

Psychologists are physicians who study the human mind and human behavior to help individuals cope with illnesses or injuries.
Chaplains offer spiritual and emotional care to patients, families and staff, particularly during times of crisis. This care includes a listening presence, encouragement, resources for spiritual and religious needs, as well as assistance in contacting leaders of the patient's own faith community.
Dietitians assess patients’ nutritional needs, develop and implement nutrition programs.
The Unit
The Center for Rehabilitation at Westlake Hospital is an acute, physical rehabilitation unit offering a wide range of services to meet the needs of patients looking to recover their highest level of function. Our team of experts are dedicated to helping each patient increase their independence, adapt to new challenges, and restore self-determination.
Discharge
In anticipation of a patient’s return home, our rehabilitation team is involved in the discharge planning process, home assessment and homecare. The recovery process continues beyond discharge, as many patients return to Westlake Hospital for Outpatient Rehabilitation.
Referral Process
Patients are referred to the Center for Rehabilitation from a variety of sources, including physicians, insurance carriers, social workers, home health professionals, employers and family members.
