The Medical Arts Pharmacy provides high quality prescription services for employees, patients and the general public. Pharmacists are available to speak with customers directly concerning individual prescription needs. Most prescription insurance cards are accepted.

Anticoagulation Clinic

Westlake Hospital Pharmacy has established an Anticoagulation Clinic in an effort to assist warfarin (Coumadin®) patients with education and management of their therapy. Because warfarin is used to treat and prevent blood clots, it is a medicine that needs to be monitored closely to make sure the dose is safe and effective.

At the clinic, a clinical pharmacist will assist you and your physician in managing your warfarin therapy to decrease the risk of harmful side effects and increase the success of treatment.

How does the clinic work?

After your doctor has referred you to the clinic, you will be contacted to schedule your first appointment. At this visit, and at follow-up visits, you will meet with the pharmacist to discuss any questions you may have concerning your therapy.

The pharmacist will need to know specific information from you such as:

  • Your current warfarin dose, including tablet strength, shape and color.
  • Other medications and supplements (i.e. vitamins, herbals) that you take whether they are prescribed or over-the-counter medicine.
  • Other medical conditions that you are being treated for by your doctor.

The pharmacist will complete a patient assessment and evaluate for potential drug interactions or issues you may encounter while on warfarin therapy.

A finger-stick blood test will be performed to determine how well your current warfarin dose is working. Based on the blood test results, necessary dosing changes will be made at each visit. You will be given a dosing card with directions on how you should take your warfarin until the next scheduled visit.

The information obtained at your visit, including your test results and dosing instructions, will be faxed to your physician. Your doctor will continually be informed and kept up-to-date regarding your warfarin therapy.

Will my insurance pay for this service?

Medicare, along with most private insurance companies, will pay for this service. For your convenience, Westlake Hospital will bill your insurance directly.

Where is the clinic?

Westlake Hospital Anticoagulation Clinic (WAC) is conveniently located in the Professional Office Building in the Medical Residents Clinic area.
